What We Offer

Chestnut Family Health Center can help you to sign up for affordable health insurance through the Illinois and Federal Health Insurance Marketplace. Help is available to anyone living in the area; it is not just for patients of the Health Center.

If you are eligible for coverage, we can help you to:

  • Enroll in Medicaid and CHIP
  • Enroll during the Marketplace special enrollment period. You qualify if you have recently gotten married, had a baby, or if a child has left home for college or for a new job.
  • Apply for an exemption from coverage.
  • Make changes to current Marketplace applications They include ending your insurance coverage, changing qualified health plans, adding dependents to your insurance, reporting a change in your income, etc.

Chestnut Family Health Center is committed to your health. At present, Chestnut Family Health Center is not offering COVID-19 testing. During the coronavirus outbreak, we have put the following guidelines into practice:

  • If you have a fever, cough, difficulty breathing or have been near someone who has tested positive for COVID-19, please do not come to our office even if you have an appointment. Please call our office for an over-the-phone evaluation by our staff. 
  • If you have concerns about coming to our office for a scheduled appointment, please call our office at least one day before your appointment to talk with staff. 
  • Avoid public places, groups of more than ten people, physically touching other people, and touching your face. Wash your hands often with soap and water for at least 20 seconds.
