Health Care Services 

We strongly believe in preventative medicine and encourage annual physical exams, gynecological care, immunization updates and cancer screenings. We are always happy to discuss lifestyle issues that may improve your health. 

Health care is available for children and adults. We treat your immediate concerns like colds, the flu and infections and ongoing conditions like high blood pressure and diabetes.

Other services include:

  • Wellness visits (immunizations, shots)
  • Work, school and sports physicals
  • DOT and other physicals
  • Women’s health
  • Pregnancy care and family planning services
  • Weight management
  • Treatment for behavioral health and wellness issues, such as depression and anxiety
  • Same-day sick visits

Our Hours: Monday, Tuesday, Wednesday, Friday: 8:00 a.m. - 5:00 p.m. and Thursday: 8:00 a.m. - 7:00 p.m.



Chestnut Family Health Center is committed to your health. At present, Chestnut Family Health Center is not offering COVID-19 testing. During the coronavirus outbreak, we have put the following guidelines into practice:


  • If you have a fever, cough, difficulty breathing or have been near someone who has tested positive for COVID-19, please do not come to our office even if you have an appointment. Please call our office for an over-the-phone evaluation by our staff. 
  • If you have concerns about coming to our office for a scheduled appointment, please call our office at least one day before your appointment to talk with staff. 
  • Avoid public places, groups of more than ten people, physically touching other people, and touching your face. Wash your hands often with soap and water for at least 20 seconds.
