Making an Appointment

Chestnut Family Health Center has two convenient centers: Bloomington and Granite City. Both provide primary medical care, laboratory services, pharmacy, screening and treatment for mental health issues, routine physicals, and OB/GYN services, regardless of your ability to pay.

Scheduling Your Appointment

To make an appointment at Chestnut Family Health Center in Bloomington, call (309) 557-1400.

Sliding Fee Discounts

Chestnut Family Health Center is a community-based health center. Access to a sliding fee schedule for patients who qualify based on family size and income is available. Chestnut's financial intake staff are happy to meet with you to review eligibility requirements and establish a sliding fee rate. 


We offer services at discounted rates for patients who qualify based on their family size and income. Co-payments are collected at the time of service. Insurance co-pays are shown on your insurance card. We accept the following insurance: 

  • Blue Cross/Blue Shield
  • Coventry
  • Health Alliance
  • Illinicare/Cenpatico
  • Meridian
  • Molina
  • Wellcare

Insurance Coordinators are available to assist with insurance eligibility questions and enrollment through the Illinois and Federal Insurance Marketplace. All services are confidential. Patients are seen regardless of insurance coverage or ability to pay.

Interpretation Services

Chestnut Family Health Center provides interpreting services, available by phone so that we can better serve our patients who do not speak English. Sign language interpretation is also available. If you need these services, please let us know 48 hours before your appointment.

Help With Transportation 

Need help getting to your appointment? Chestnut Family Health Center is here for you! Our dedicated Case Management staff can assist you in coordinating transportation to your appointments.

Chestnut Family Health Center continues to care for patients during the COVID-19 outbreak. To comply with social distancing recommendations, we are offering some appointments by telephone or video. Our in-house Genoa Pharmacy is offering curbside pick-up and mail order delivery. For patients who are seen in the office, we are doing extra cleaning and have a number of precautions in place to protect your health and ours. During the outbreak, we are following these guidelines:

  • If you have a fever, cough, difficulty breathing or have been near someone who has tested positive for COVID-19, please do not come to our office even if you have an appointment. Please call our office for an over-the-phone evaluation by our staff.
  • If you have concerns about coming to our office for a scheduled appointment, please call our office at least one day before your appointment to talk with staff.
