When you become a patient of Chestnut Family Health Center you will find that YOU are the most important part of your care team. Our goal is to give you the support you need to meet your health care needs. As a valued member of the team, you will be able to share the things that are really important to you. During your visits, your doctor and other members of the team will:

  • Listen to your concerns
  • Answer questions
  • Help you better understand your health care needs and options
  • Provide care created just for you
  • Coordinate your care with both Chestnut and non-Chestnut providers
  • Encourage you to be active in your own health care

You can always reach a member of your health care team. During non-working hours, call (309) 557-1400 for the Bloomington health center. Call (618) 512-1919 for the Granite City health center. A nurse is available by telephone. You may access your medical information by signing up for the patient portal.

Chestnut Family Health Center is committed to your health. At present, Chestnut Family Health Center is not offering COVID-19 testing. During the coronavirus outbreak, we have put the following guidelines into practice:

  • If you have a fever, cough, difficulty breathing or have been near someone who has tested positive for COVID-19, please do not come to our office even if you have an appointment. Please call our office for an over-the-phone evaluation by our staff.
  • If you have concerns about coming to our office for a scheduled appointment, please call our office at least one day before your appointment to talk with staff.
  • Avoid public places, groups of more than ten people, physically touching other people, and touching your face. Wash your hands often with soap and water for at least 20 seconds.

We value our patients and want to ensure their safety. Thank you for your cooperation.

If you feel that you have a medical emergency, please call 911 or go to one of the local hospital Emergency Departments.
