For both the Bloomington and Granite City locations, hours are:

Monday, Tuesday, Wednesday, and Friday 8:00 a.m. – 5:00 p.m.

Thursday 8:00 a.m. – 7:00 p.m.

Which insurance do you accept?

We accept all forms of Medicaid. In addition, we accept Medicare, Blue Cross Blue Shield, Health Alliance, and Country Healthcare.


Do you offer services at reduced fees?

Chestnut Family Health Center offers a sliding fee schedule for patients who qualify based on family size and income. Our staff members will be happy to meet with you to see if you are eligible. 


What services do you offer?

Chestnut Family Health Center cares for patients of all ages, from birth through end of life. A list of some of the types of conditions we can treat appears below. If you have specific questions, please give us a call.

  • Wellness visits
  • Work, school, and sports physicals
  • DOT and other physicals (list which location)
  • Women’s health
  • Pregnancy care and family planning (list which location)
  • Weight management
  • Management of ongoing conditions such as diabetes and high blood pressure
  • Depression
  • Anxiety
  • Same-day sick visits


What do I need to bring to my appointment?

  • Past medical history
  • Current medications (please bring your medication bottles to each appointment)
  • Pharmacy information
  • Picture ID
  • Insurance card
  • Immunization records (if available)
  • Proof of income (if currently uninsured)


Chestnut Family Health Center is committed to your health. At present, Chestnut Family Health Center is not offering COVID-19 testing. During the coronavirus outbreak, we have put the following guidelines into practice:


  • If you have a fever, cough, difficulty breathing or have been near someone who has tested positive for COVID-19, please do not come to our office even if you have an appointment. Please call our office for an over-the-phone evaluation by our staff. 
  • If you have concerns about coming to our office for a scheduled appointment, please call our office at least one day before your appointment to talk with staff. 
  • Avoid public places, groups of more than ten people, physically touching other people, and touching your face. Wash your hands often with soap and water for at least 20 seconds.
